About the Role:

The Licensed Practical Nurse (LPN) at Elwood Skilled Nursing Facility (SNF) plays a critical role in delivering high-quality nursing care to residents in a long-term care environment. This position involves providing direct resident care, monitoring patient health, and assisting with the implementation of individualized care plans under the supervision of registered nurses and physicians. The LPN will ensure the comfort, safety, and well-being of residents by administering medications, performing routine clinical tasks, and documenting patient progress accurately. Collaboration with interdisciplinary teams is essential to coordinate care and support the facility’s commitment to compassionate and effective healthcare. Ultimately, the LPN contributes to enhancing the quality of life for residents by maintaining professional standards and adhering to regulatory guidelines within the SNF setting.

Minimum Qualifications:

  • Current and valid Licensed Practical Nurse (LPN) license in the state of employment.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated knowledge of nursing principles, medication administration, and patient care standards.
  • Ability to communicate effectively with residents, families, and healthcare team members.

Preferred Qualifications:

  • Experience working in a skilled nursing facility or long-term care environment.
  • Additional certifications such as IV therapy or wound care.
  • Familiarity with electronic health record (EHR) systems.
  • Strong organizational and time management skills.
  • Continuing education in geriatric nursing or related specialties.

Responsibilities:

  • Administer prescribed medications and treatments to residents safely and accurately.
  • Monitor and record vital signs, symptoms, and changes in residents’ conditions, reporting significant findings to the nursing team.
  • Assist with daily living activities such as bathing, dressing, and mobility support to promote resident independence and comfort.
  • Collaborate with registered nurses, physicians, and other healthcare professionals to develop and update individualized care plans.
  • Maintain detailed and accurate documentation of nursing care, medication administration, and resident progress in compliance with facility policies and regulatory requirements.
  • Respond promptly to resident needs and emergencies, providing compassionate and effective care.
  • Participate in staff meetings, training sessions, and quality improvement initiatives to enhance nursing practices and resident outcomes as assigned.

Skills:

The LPN utilizes clinical skills daily to assess resident health, administer medications, and perform nursing procedures with precision and care. Effective communication skills are essential for interacting with residents, families, and interdisciplinary team members to ensure coordinated and personalized care. Critical thinking and problem-solving abilities enable the LPN to recognize changes in resident conditions and respond appropriately. Organizational skills support accurate documentation and efficient management of multiple tasks in a fast-paced environment. Preferred skills such as proficiency with electronic health records and specialized certifications enhance the LPN’s ability to deliver comprehensive care and contribute to continuous quality improvement within the facility.
