About the Role
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team on a part-time / PRN basis. This role is ideal for nurses who value autonomy, flexible scheduling, and one-on-one patient care in the home setting. As a Home Health RN, you will provide skilled nursing services, patient education, and coordinated care to support patients in achieving optimal health outcomes.
Responsibilities
- Perform in-home patient assessments and develop individualized care plans.
- Provide skilled nursing care including wound care, medication management, catheter care, and other treatments as ordered.
- Monitor and document patient progress, changes in condition, and response to treatment.
- Educate patients and caregivers on disease processes, medications, and self-care techniques.
- Communicate effectively with physicians, case managers, therapists, and other members of the care team.
- Complete timely and accurate clinical documentation in accordance with agency policies and regulatory standards.
- Participate in on-call rotation (if applicable and agreed upon).
- Ensure compliance with all state, federal, and home health regulations.
Qualifications
- Current, active RN license in the state of Michigan.
- Minimum 1 year of nursing experience; home health experience preferred but not required.
- Current CPR/BLS certification.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Ability to work independently and manage time effectively.
- Strong clinical judgment, communication skills, and attention to detail.
- Comfortable using EMR systems for documentation (training available).

Pay: $45.00 - $85.00 per hour
