RN – In-Home Hospice

Position Summary
The Registered Nurse (RN) provides compassionate, end-of-life care to patients in their homes. The RN assesses, plans, implements, and evaluates care focused on comfort, dignity, and quality of life for patients and their families.
Key Responsibilities
- Provide skilled nursing care in patients’ homes following physician orders
- Assess physical, emotional, and spiritual needs of hospice patients
- Develop and update individualized plans of care
- Administer medications and manage pain and symptom control
- Perform admissions, routine visits, and reassessments
- Educate patients and families on disease process and end-of-life care
- Document care in the electronic health record accurately and timely
- Coordinate care with the interdisciplinary team (physicians, social workers, aides, etc.)
- Respond to patient/family needs with compassion and urgency
Typical Work Setting
- Patient homes (primary)
- Sometimes nursing homes or assisted living facilities
- Travel within a regional service area (often multiple counties)
Schedule
- Full-time or part-time
- May include:
- Weekends
- On-call rotation
- Holidays
Qualifications
- Active Michigan RN license
- Graduate of accredited nursing program
- 1–3 years nursing experience (hospice, home health, or acute care preferred)
- Valid driver’s license and reliable transportation
- Strong communication and critical thinking skills
