Schedule: PRN / Part-Time (flexible scheduling)
Location: New Buffalo, Michigan
About the Role
We are seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our home health team on a part-time basis. The PTA will work under the direction and supervision of a licensed Physical Therapist to provide in-home therapeutic care that helps patients regain strength, mobility, and independence within the comfort of their own homes.
Responsibilities
- Provide in-home physical therapy treatments as established in the patient’s plan of care developed by the supervising Physical Therapist.
- Assist patients with therapeutic exercises, gait training, balance activities, home safety training, and functional mobility.
- Observe, document, and report changes in patient condition, response to treatment, and any concerns to the supervising PT.
- Educate patients and caregivers on exercises, mobility techniques, and home safety to support continued progress.
- Communicate effectively with the interdisciplinary team, including PTs, nurses, OTs, home health aides, and case managers.
- Accurately complete all required documentation in the EMR in accordance with Medicare and state home health regulations.
- Maintain compliance with Michigan licensure standards and Home Health Agency policies.
- Participate in team meetings, in-services, and training as needed.
Qualifications
- Current Physical Therapist Assistant (PTA) license in the State of Michigan (required).
- Graduation from an accredited PTA program.
- Home health experience is preferred, but new grads with strong clinical skills will be considered.
- Current CPR/BLS certification.
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong communication, documentation, and time-management skills.
- Ability to work independently in the field and adapt to a flexible schedule.

Pay: $35.00 - $45.00 per hour
