Value-Based Care Coordinator
We’re seeking a Value-Based Care Coordinator to support our value-based programs with ACOs, Medicare Advantage Plans, and other payer partners. You’ll help ensure contracts are executed successfully, performance metrics are tracked, and communication flows smoothly between internal teams and external partners.
What You’ll Do:
- Support implementation and management of value-based care programs
- Track quality measures, performance metrics, and contract deliverables
- Prepare reports and presentations for leadership and partners
- Coordinate between clinical, operational, and payer teams
- Help improve patient outcomes through population health initiatives
What We’re Looking For:
- Bachelor’s degree or equivalent experience in healthcare, business, or related field
- Experience in managed care, healthcare operations, or value-based programs preferred
- Strong analytical, organizational, and communication skills
- Proficiency with Excel and comfort working in EHRs and payer portals
- Detail-oriented with a compliance mindset
