Aging Partners is looking for a Community Network Specialist – Medicaid Waiver to build partnerships, educate community agencies, and ensure that older adults and individuals with disabilities can access the Aged and Disabled (AD) Waiver Program with confidence and clarity.
In this role, you’ll be the bridge between Aging Partners, community organizations, and state agencies—helping to strengthen understanding, improve service coordination, and advocate for equitable access to care. This is a supervisory role providing leadership to assigned Medicaid Waiver Service Coordinators.
What You’ll Do
- Build and maintain strong relationships with community agencies, providers, and stakeholders.
- Educate partners about Aged and Disabled (AD) Waiver Services through outreach, trainings, and accessible materials.
- Collaborate with internal teams and the Eligibility & Enrollment Unit to improve communication and client support.
- Lead and mentor staff, promoting professionalism, teamwork, and accountability.
- Review staff casework to ensure quality service and compliance with program standards.
- Represent Aging Partners at appeal hearings and serve as a knowledgeable program advocate.
- Foster collaboration across departments and strengthen relationships with local and state partners.
- Stay informed on aging and disability trends and provide community trainings as needed.
Work Schedule
Monday - Friday. 8 a.m. to 4:30 p.m.
This position's starting pay is $30.13 per hour.
Minimum Qualifications
Graduation from an accredited four-year college or university with major coursework in social work, gerontology, psychology, human services or related field plus two years of experience in providing program services or case management to a geriatric or disabled population including six months of supervisory experience or any equivalent combination of training and experience that provides the desirable knowledge, abilities and skills.
*A valid Driver's License is strongly preferred
