Summary:
The Child and Family Care Coordinator provides comprehensive, family-centered care coordination for children and adolescents receiving outpatient assessment and treatment for Fetal Alcohol Spectrum Disorders (FASD), Autism Spectrum Disorder (ASD), developmental disabilities, behavioral health conditions, and other neurodevelopmental needs.
The Care Coordinator serves as a primary point of contact for children, parents, caregivers, foster families, and multidisciplinary providers. The position assists families in navigating complex medical, behavioral health, developmental, educational, social service, and community systems and ensures that recommendations resulting from multidisciplinary assessments are translated into coordinated services and individualized plans of care.
The Child and Family Care Coordinator works closely with the Child and Family Developmental services and multidisciplinary team to provide coordination and contact for children and families.
Major Responsibilities:
- Serve as the primary care coordination contact for children and families enrolled in the FASD and Autism outpatient program.
- Conduct initial care coordination assessments to identify the child's and family's medical, developmental, behavioral, educational, social, and practical needs.
- Work collaboratively with families to identify priorities, strengths, concerns, and treatment goals.
- Develop, maintain, and update individualized Child and Family Plans of Care.
- Help families understand diagnoses, assessment findings, provider recommendations, and next steps.
- Track referrals and follow-up recommendations to reduce gaps in services.
- Provide ongoing contact with families to identify barriers to treatment participation.
- Coordinate transitions between assessment, diagnosis, outpatient treatment, community services, and other levels of care.
- Coordinate services among pediatric, psychological, psychiatric, speech-language, occupational therapy, physical therapy, behavioral health, educational, and community providers.
- Participate in multidisciplinary case conferences and assist the team in developing integrated recommendations and treatment plans.
- Maintain communication among providers so families receive consistent information and understand which team member is responsible for each component of the child's treatment plan.
- Coordinate with schools, Head Start programs, early intervention providers, and special education teams.
- Assist families with navigating developmental and educational services.
- Provide families with understandable information about FASD, autism, developmental disabilities, and available treatment and support services.
- Support access to services for children and families living in rural and communities.
- Promote communication between outpatient clinicians and educational providers.
- Coordinate services for children involved with foster care, kinship care, adoption, or child welfare systems.
- Screen for or identify barriers that may interfere with the family's ability to participate in treatment, including transportation, housing instability, food insecurity, financial limitations, childcare, technology access, caregiver stress, and difficulty navigating service systems.
- Connect families with appropriate Tribal, state, federal, nonprofit, educational, and community resources.
- Maintain timely and accurate documentation in the electronic health record or designated care management system.
- Document family contacts, referrals, service needs, barriers, care plans, and follow-up activities.
- Support program data collection and quality-improvement activities.
- Performs other duties as assigned or required.
- Bachelor's degree in social work, psychology, human services, child development, behavioral health, public health, nursing, education, rehabilitation services, family studies, or a closely related field.
- Experience working with pediatric, developmental, neurological, or neurodevelopmental populations preferred.
- Two or more years of experience providing care coordination, case management, family navigation, or related services to children and families.
- Experience providing services in rural, Tribal, underserved, or culturally diverse communities preferred.
- Willingness to promote continuous quality improvement activities.
- Willingness to follow Chugachmiut employee values. Interest in and knowledge of Alaskan cultures (in particular Alaskan Native cultures).
- No DWI or other major alcohol or other drug violations within the last 36 months.
