Edgehill Nursing and Rehabilitation is seeking a Community Care Coordinator professional to join our team!
This position combines Admissions and Social Services functions which include managing referrals, conducting tours, responding to inquiries, and complete pre-admission and admission paperwork ensuring regulatory compliance. Support census development through marketing and sales related activity to achieve occupancy and payer mix goals, and other related revenue generating practices in conjunction with census development and bed management.
Coordinating the care and services of incoming clients by performing aspects of the external liaison and education process with oversight of the internal aspects of the inquiry process and admissions process through standardized practices and procedures.
Supporting residents and family members through counseling, discharge planning, and ongoing education, while maintaining policies and procedures according to expected standards. Planning, organizing, developing, administering, and evaluating the social services program for the residents, discharge planning, and overall operation of the social service department.
Monitoring and ensuring that the psycho-social needs of residents are assessed, addressed, and tracked, assures the delivery of and documentation of Social Services congruent with current regulations; and facilitates delivery of services and advocating resident rights.
