Job Overview
Supportive Home Care Services is seeking a passionate, outgoing, and motivated Community Relations Coordinator to represent our agency across Philadelphia, Bucks, Chester, Delaware, and Montgomery counties. This role is ideal for someone who enjoys building relationships, educating the community, and connecting individuals in need with compassionate, in-home care services. This role requires a proactive approach to problem-solving, as well as a keen understanding of marketing strategies and analytics.

Responsibilities
- Establish and maintain relationships with referral sources, including hospitals, social workers, discharge planners, senior centers, physician offices, skilled nursing facilities, and community organizations.
- Conduct in-services and presentations to educate referral sources and community partners about home care services.
- Represent the company at health fairs, community events, networking meetings, and senior-focused programs.
- Track leads and referrals, follow up with prospects, and coordinate initial intake visits to ensure a smooth onboarding process for new clients.
- Monitor referral outcomes and maintain activity reports, as well as marketing performance data.
- Stay up to date with industry trends and local community needs..
Qualifications
- 2 years of experience in healthcare marketing, home care, senior living, or community outreach.
- Strong communication and interpersonal skills; comfortable speaking with professionals and families alike.
- Ability to travel throughout Philadelphia and surrounding counties.
- Self-starter with the ability to work independently and manage time effectively.
- Bachelor's degree in Marketing, Healthcare Administration, Social Work, or related field preferred.
- Valid driver’s license and reliable transportation required.
Commission Structure for Referral Bonuses:
- $200 for clients who continue services for 4 weeks (20 hrs/week)
- $300 for long-term clients active for 8 weeks (20 hrs/week)
