If yes, here’s how you will contribute to the team:
As a Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN), you will put resident service first and ensure that residents and their families receive the highest quality care. Under the direction of a registered nurse, you will deliver a full range of nursing care. This includes, but is not limited to:
- Admitting, transferring, and discharging residents.
- Conducting daily rounds to observe the resident’s physical and emotional status.
- Medication administration.
- Monitoring resident care provided by unlicensed staff.
- Maintaining the confidentiality of all resident care information.
- Promoting a culture of safety to ensure a healthy practice and living environment.

Must Have:
- Graduate of a National League for Nursing accredited school of nursing
- Must have minimum 1 year of experience as an LVN field nurse in Home Health
- Experience in EMR software (preferred).
- Provide skilled nursing services / interventions in accordance with physician orders.
- Assesses patient needs, plans for, evaluates and modifies care to meet goals of skilled nursing interventions.
- Collaborates with all disciplines to plan and evaluate team goals for each patient.
- Wound Care experience
Required Licenses and Certifications:
- LVN license in CA
- Current CPR certification
- Valid Driver’s License and Auto Insurance
- Physical/TB from the last 6 months or sooner from hire
