Location: In-Person supporting our St. Cloud Assertive Community Treatment (ACT) within a four (4) county region (Stearns, Benton, Sherburne, & Wright). Office is located in Waite Park, MN
Central Minnesota Mental Health Center (CMMHC) has a rewarding opportunity to serve as a Psychiatric Mental Health Nurse Practitioner (PMHNP) within our St. Cloud Assertive Community Treatment Team. The PMHNP is responsible for prevention, diagnosis, and treatment of mental disorders of clients in an outpatient setting through interview, assessment, observation, and screening tools. The PMHNP works in collaboration with agency mental health professionals, clinicians, case managers, and nurses.

Assertive Community Treatment (ACT) is an intensive, non-residential mental health service for persons with serious mental illness. These individuals commonly experience multiple barriers to successful functioning, including co-occurring substance abuse or dependence, homelessness and unemployment. ACT teams help individuals treat and manage their mental illnesses and develop the skills they need for life in the community of their choice.

ACT Services Are
- Provided by multidisciplinary team
- A blend of treatment, rehabilitation, therapeutic, and supportive services
- Directed to persons with a serious mental illness who require intensive services
- Offered on a time-unlimited basis and available 24 hours per day, 7 days per week, 365 days per year.
- Provide diagnostic and psychotherapeutic care to clients
- Provides clinical oversight of ACT services in conjunction with Team Lead
- Prescribes, directs, and administers psycho-therapeutic treatments of medications to treat mental, emotional, or behavioral disorders
- Responds appropriately to psychiatric crisis situations.
- Maintains ethical and confidential standards.
- Orders, conducts, or examines laboratory or diagnostic tests on patients to provide information on general physical condition and mental disorder gathers and maintain patient information and records, including social and medical history obtained from patients, relatives, and other professional
- Collaborates with team leader in sharing overall clinical responsibility for screening and admitting clients, monitoring clients’ treatment and service delivery and providing clinical supervision to the team.
- Educates staff on psychiatric and non-psychiatric medication, side effects and health related conditions
- Actively collaborates with nursing staff related to clients’ treatment and service delivery
- Active team member on ACT team providing diagnostic assessments, medication management and medication education, treatment plans, and consultative services for ACT clients.
- Conducts home and community-based client visits
- Serves as the point of contact for psychiatric treatment when a client is hospitalized for mental health treatment and shall communicate directly with the client’s inpatient psychiatric care providers to ensure continuity of care.
Required Experience & Skills:
- Completion of Graduate level Advance Practice Registered Nurse (APRN) program with a focus in Psychiatric and Mental Health
- Maintain appropriate licensure, including completing continuing education licensing requirements
- Must have demonstrated clinical experience working with individuals with serious and persistent mental illness (SPMI)
- Clear DHS background study
- 3 years’ community mental health experience preferred.
