- Monday – Thursday: 7:30 AM – 6:00 PM
- Friday: 7:30 AM – 12:30 PM

How You’ll Make An Impact
The Care Coordinator is responsible for coordinating the care for clients and/or families receiving services, promoting quality care, and supporting collaboration for clients. The Care Coordinator reports to the Director of Access
- Provide culturally appropriate care coordination for clients as needed, coordinating with referral source to assess best services for client.
- Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services, and promote client autonomy.
- Maintain current knowledge of the mental health service system with the ability to locate information and resources for a variety of client needs, including medical, housing, employment, social services, and community programs.
- Assist clients with insurance
- Complete all required documentation by specified deadline
- Provide support and coordination for clients who are on the Adult Clinical Care Depression Pathway
- Assist with treatment planning process by coordinating treatment planning meetings and assisting in goal achievement.
- Assist clients in addressing barriers to accessing services, including transportation, insurance, finances, and legal issues.
- Attend weekly team meetings in assigned location to collaborate with providers.
- Meet with Director of Access monthly
- Assist support staff and clinicians in identifying appropriate referrals and resources for clients or potential clients
- Responsible for care coordination, whether it involves coordination within CMMHC’s programs or with another entity identified in the Certified Community Behavioral Health Clinic (CCBHC) statutory language related to care coordination.
WHAT YOU’LL NEED
- High School Diploma or GED required
- Clear DHS Background Study.
- Valid Driver’s License with acceptable driving record.
- 2 or more years of Administrative and Customer Service experience in a client-facing role preferred
- Experience with multiple phone lines preferred
- Associate’s degree in medical office administration preferred
- Outgoing personality and skilled at building relationships.
- Flexibility and tolerance for consistent change throughout your work week.
- Ability to work independently.
- Knowledge of community and area resources.
