Castle Rock Home Care is seeking a dedicated Service Coordinator to support applicants and participants in the NHTD/TBI Waiver Program. This role involves providing case management, advocacy, and coordination of services to ensure participants receive the care and support they need.
Responsibilities
- Develop and implement Initial Service Plans
- Coordinate services between providers and participants
- Conduct monthly in-home visits
- Facilitate, orchestrate, and document team meetings
- Provide ongoing participant support and advocacy
- Prepare and submit Renewal Service Plans
- Master's degree and 1 year of case management experience, OR Bachelor’s degree with 3 years of case management experience, OR Associate’s degree with 5 years of case management experience
- Ability to travel throughout the five boroughs (public transit or personal vehicle)
- Bilingual English/Spanish strongly preferred
- Strong organizational and communication skills
Salary : $55,000 - $75,000
