We are seeking a motivated and outgoing Home Care Field Marketer to develop and maintain referral sources throughout New York City. This role focuses on building relationships with healthcare professionals, community organizations, and facilities to generate client referrals for home care services.

Key Responsibilities
- Develop and maintain relationships with referral sources including hospitals, rehab centers, nursing homes, and social workers
- Promote home care services to discharge planners, case managers, and community organizations
- Educate referral partners on services offered through Licensed Home Care Services Agencies (LHCSAs)
- Generate and track new patient referrals and admissions
- Conduct in-person visits across NYC boroughs (Brooklyn, Queens, Bronx, Manhattan, Staten Island)
- Represent the agency at community events, health fairs, and senior centers
- Collaborate with intake and coordination teams to ensure smooth patient onboarding
- Maintain accurate records of outreach activities and referral pipelines
- Ensure all marketing practices comply with HIPAA and New York State Department of Health regulations
- High school diploma required; Associate’s or Bachelor’s degree preferred
- 1 year of experience in healthcare marketing, home care, or medical sales (NYC experience strongly preferred)
- Existing relationships with MLTC plans, hospitals, or nursing facilities is a major plus
- Knowledge of Medicaid home care services and referral processes in NYC
- Strong interpersonal and networking skills
- Self-motivated with the ability to work independently in the field

- Pay: 65,000 - 125,000 a year
- Performance-based bonuses/commissions (based on referrals/admissions)
