Position Overview

The Social Services Director is responsible for planning, organizing, and directing the social service program within the facility. This role serves as an advocate for residents, helping them and their families navigate the emotional, social, and practical aspects of long-term care while ensuring residents' rights and quality of life remain a top priority.

Responsibilities

• Complete psychosocial assessments and develop individualized care plans.
• Assist residents and families with admissions, transitions, and discharge planning.
• Coordinate resident care conferences and family meetings.
• Advocate for residents' rights and promote resident satisfaction.
• Connect residents and families with community resources and support services.
• Maintain accurate documentation in accordance with state and federal regulations.
• Collaborate with nursing, therapy, dietary, activities, and other interdisciplinary team members.
• Assist with Advance Directives, guardianship issues, and other resident needs as appropriate.
• Promote a positive, welcoming environment for residents and visitors.

Qualifications

• Previous experience in social services within a healthcare or long-term care setting preferred.
• Bachelor's degree in Social Work or a related human services field preferred.
• Knowledge of long-term care regulations is a plus.
• Excellent communication and interpersonal skills.
• Compassionate, organized, and resident-focused.
• Ability to work independently while collaborating with an interdisciplinary team.
