Cass County Senior Living & Rehabilitation is seeking an experienced and motivated Registered Nurse to join our clinical leadership team in a unique dual-role position as MDS Nurse / Infection Preventionist. Position Overview

This position combines the responsibilities of both the MDS Nurse and Infection Preventionist, allowing you to play an essential role in resident care, quality outcomes, and regulatory compliance.

Your weekly schedule will be divided between both areas of responsibility, with participation in the facility's on-call rotation.

Responsibilities

MDS Responsibilities

• Complete and coordinate MDS assessments in accordance with state and federal regulations.

• Ensure accurate coding to support reimbursement and quality measures.

• Develop and update resident care plans.

• Collaborate with the interdisciplinary team to ensure individualized resident care.

• Maintain accurate and timely documentation.

Infection Prevention Responsibilities

• Lead the facility's Infection Prevention and Control Program.

• Conduct surveillance, tracking, and reporting of infections.

• Audit infection control practices throughout the facility.

• Educate staff on infection prevention standards and best practices.

• Monitor compliance during all shifts, including periodic off-shift observations.

• Ensure compliance with CDC, CMS, and IDPH infection prevention guidelines.

Qualifications

• Active Illinois Registered Nurse (RN) License required.

• Previous MDS experience required.

• Infection Prevention experience preferred but not required.

• Willingness to complete the CDC Infection Preventionist Training if not already certified.

• Strong assessment, organizational, and communication skills.

• Ability to work independently while collaborating with an interdisciplinary team.

• Participation in the facility's on-call rotation required.
