Position Summary
We are seeking a dedicated and compassionate Physical Therapist to join our interdisciplinary care team. The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions designed to improve mobility, strength, balance, safety, and overall functional independence.
Essential Responsibilities
- Evaluate residents' physical abilities, limitations, and rehabilitation needs.
- Develop and implement individualized treatment plans based on physician orders and resident goals.
- Provide therapeutic exercises, gait training, transfer training, balance activities, and mobility interventions.
- Educate residents, families, and caregivers on treatment goals, home exercise programs, and fall prevention strategies.
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals.
- Monitor resident progress and modify treatment plans as appropriate.
- Complete timely and accurate documentation in accordance with facility, Medicare, Medicaid, and regulatory requirements.
- Participate in care planning meetings and contribute to resident-centered care initiatives.
- Ensure compliance with all state, federal, and facility policies and procedures.
Qualifications
- Current and unrestricted Physical Therapist license in the state of South CArolina
- Current CPR certification.
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively with geriatric and medically complex populations.
Preferred Qualifications
- Experience in long-term care, skilled nursing, rehabilitation, or geriatric settings.
- Knowledge of Medicare Part A and Part B rehabilitation services.
- Experience with electronic medical records (EMR).
- Certification in geriatric physical therapy or related specialty preferred.
