Summary of Responsibilities:
- Schedule and oversee scheduling of all resident MDS assessments to assure timeliness.
- Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements.
- Participate in the pre-admission process to determine residents’ level of care.
- Participate in resident level of care determination upon admission.
- Participate in tracking changes in residents’ level of care to assure timely scheduling of MDS assessments.
- Be familiar with residents’ condition and care needs.
- Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections.
- Review reports of timely completion of residents’ MDS assessments and care plans and take follow-up action as necessary.
- Participate in the Quality Assessment and Assurance Program, for the nursing department, in cooperation with the Quality Assurance Coordinator.
- Analyze Quality Indicator reports for potential resident concerns and problems and review residents’ care plans to be sure this information is included.
- Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings.
- Good organizational and customer service skills
- Valid Registered Nurse or Licensed Practical Nurse and CPR certification
- Desire to work with older adults
