MDS Registered Nurse - Florence

Carlyle Senior Care of Florence is seeking a Registered Nurse to join our team as the MDS Coordinator! Our goal is to provide the best care to the residents of our community every day.

Summary of Responsibilities:

• Schedule and oversee scheduling of all resident MDS assessments to assure timeliness.
• Coordinate and oversee all functions of the MDS assessment process to assure compliance with federal and state requirements.
• Participate in the pre-admission process to determine residents’ level of care.
• Participate in resident level of care determination upon admission.
• Participate in tracking changes in residents’ level of care to assure timely scheduling of MDS assessments.
• Be familiar with residents’ condition and care needs.
• Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections.
• Review reports of timely completion of residents’ MDS assessments and care plans and take follow-up action as necessary.
• Participate in the Quality Assessment and Assurance Program, for the nursing department, in cooperation with the Quality Assurance Coordinator.
• Analyze Quality Indicator reports for potential resident concerns and problems and review residents’ care plans to be sure this information is included.
• Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings.

• Good organizational and customer service skills
• Valid Registered Nurse or Licensed Practical Nurse and CPR certification
• Desire to work with older adults
