Responsibilities

• Deliver comprehensive nursing care to residents, including medication administration, wound care, and vital signs monitoring.
• Conduct thorough patient assessments and develop individualized care plans aligned with best practices and regulatory standards.
• Collaborate with interdisciplinary teams to coordinate treatments such as IV infusions, catheterizations, and oxygen therapy.
• Maintain accurate medical documentation using electronic health records (EHR/EHR systems), ensuring compliance with HIPAA and other privacy laws.
• Respond promptly to acute changes in resident conditions, utilizing triage skills and critical care procedures when necessary.
• Educate residents and their families about health conditions, medication management, and discharge planning.
• Adhere strictly to infection control protocols, aseptic techniques, and safety procedures to promote a safe environment for all.

Qualifications

• Valid state licensure as an LPN or RN with current certification in Basic Life Support (BLS).
• Demonstrated experience working in long-term care or nursing home settings with geriatric populations.
• Proficiency with electronic health records (EHR/EHR systems) such as Cerner or Athenahealth is preferred.
• Knowledge of complex medical procedures including IV insertion, suturing, dialysis support, and wound care is highly desirable.
• Strong understanding of medical terminology, anatomy, physiology, and pharmacology relevant to senior care.
• Ability to perform physical examinations, vital signs assessments, and patient evaluations accurately.
• Excellent communication skills with a positive attitude toward teamwork and resident-centered care.
