Responsibilities
- Deliver comprehensive nursing care to residents, including medication administration, vital signs monitoring, and physical assessments.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident’s needs.
- Manage complex clinical procedures such as IV infusions, catheterizations, wound care, and tube feeding with precision and adherence to safety protocols.
- Conduct triage assessments for incoming residents or visitors, prioritizing urgent needs and coordinating appropriate interventions.
- Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and confidentiality policies.
- Monitor residents’ conditions closely, recognizing signs of acute pain or infection and escalating concerns promptly.
- Educate residents and their families about health management strategies, including dementia care, Alzheimer’s support, and medication regimens.
Requirements
- Valid LPN or RN licensure in good standing with the state licensing board.
- Proven experience working in long-term care facilities or skilled nursing settings; experience with geriatrics highly preferred.
- Strong knowledge of medical terminology, anatomy, physiology, and long-term care regulations.
- Hands-on experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate documentation.
- Competency in clinical procedures including IV insertion, venipuncture, sterile technique, first aid, and basic life support (BLS).
- Ability to perform comprehensive patient assessments and vital signs monitoring efficiently.
- Excellent communication skills for caregiver collaboration and client interaction; sensitivity when working with individuals with disabilities or dementia.
