Physical Therapist In Home Health
Role: Conducts patient assessments to evaluate functional status, mobility, and medical conditions. Develops and implements individualized treatment plans to improve or maintain patient well-being, monitoring progress and adjusting care as needed. Provides therapy services within legal scope, collaborates with healthcare team, and educates patients and families about care plans and progress. Completes evaluations, progress notes, and reports outcomes, ensuring compliance with state guidelines. Demonstrates ability to work effectively in diverse home environments.
Qualifications: Current licensure in physical therapy, valid driver's license, vehicle insurance, or access to transportation. Preferred CPR certification within 90 days of hire.
