Role: Infusion Nurse Educator, a licensed Registered Nurse responsible for clinical education, training, and direct patient care to facilitate patient transitions to infusion services.
- Responsibilities: Educate patients, caregivers, and facility staff on OCH services and products for successful in-home or alternate site care. Conduct patient assessments to determine eligibility for OCH care. Partner with the sales team to support clinical transitions and improve patient transition volume. Coordinate with healthcare facilities and internal teams for timely patient discharge and therapy education. Ensure patient safety, compliance, and confidentiality while documenting education and assessments accurately.
