Role: Full-Time Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) providing patient care at multiple health locations.
Responsibilities: Administer medications, evaluate patient conditions, monitor vital signs, conduct assessments, manage healthcare access, perform medication reviews, and develop mental health screenings. Maintain accurate electronic health records and communicate effectively with patients and healthcare teams.
Qualifications: Active state LVN/LPN license, successful NCLEX-PN completion, experience with phlebotomy, vaccines, and point-of-care testing. Strong communication, organizational, problem-solving, and critical thinking skills. Proficiency with EMR systems, preferably Epic. Educational requirements include a high school diploma or GED and graduation from an approved nursing program.
