Role: The Case Manager is responsible for overseeing patient hospitalization from preadmission to one-week post-discharge, ensuring coordinated care to achieve optimal outcomes.

- Evaluate and assess patients' homegoing needs, facilitating safe transitions and discharges.
- Coordinate medical care and services with healthcare teams and community resources.
- Identify and connect families with community resources, adapting care plans based on evolving patient needs.
- Develop and implement educational materials for patients, families, caregivers, and staff related to discharge and transition planning.
- Display leadership and strong communication skills to enhance team collaboration and resolve conflicts.
- Provide comprehensive care addressing physical, psychosocial, educational, and safety needs across all age groups.
- Perform additional duties as required.
- Experience in healthcare; prior case management preferred.
- Excellent oral and written communication skills.
- Proficiency in MS Office and EMR systems such as Epic.
- Ability to interpret data and move freely within the hospital environment.
- Graduate of an accredited nursing school; current Ohio nursing license.
- Bachelor’s degree in nursing required; master’s preferred.
- Case management certification is preferred; BLS certification required.
- 3-5 years of relevant healthcare experience; no supervisory experience needed.
