Role Description The Case Manager is a full-time, on-site role based in Akron, OH, responsible for coordinating and delivering behavioral health and supportive services to individuals receiving care. Day-to-day responsibilities include assessing client needs, developing and monitoring individualized service plans, and connecting clients to appropriate clinical services, community resources, and benefits. The Case Manager maintains regular contact with clients through meetings and outreach, provides advocacy and support, and collaborates with treatment teams, families, and community partners to promote recovery and stability. This role also involves maintaining accurate documentation, tracking progress toward goals, and ensuring services are delivered in alignment with agency policies, confidentiality standards, and regulatory requirements.

Qualifications

  • Experience in behavioral health or social services, including client assessment, service planning, and care coordination.
  • Strong interpersonal skills, with the ability to build trusting relationships, communicate clearly, and collaborate with multidisciplinary teams and community partners.
  • Organizational and time-management skills to handle multiple cases, maintain accurate records, and meet documentation and reporting requirements.
  • Ability to work respectfully and effectively with individuals from diverse backgrounds, using trauma-informed and recovery-oriented approaches.
  • Knowledge of community resources, social service systems, and behavioral health supports relevant to Summit County and surrounding areas.
  • Proficiency with electronic health records and basic computer applications (e.g., email, word processing, data entry).
  • Bachelor’s degree in social work, psychology, counseling, or a related human services field; relevant licensure or certification is preferred where applicable.
  • Commitment to the agency’s mission and values, including confidentiality, ethical practice, and continuous improvement in service quality.
