Physical Therapist (PT)

Role Overview: Responsible for evaluating, developing, and implementing individualized physical therapy treatment plans to improve patient mobility and function. Provides direct therapy services across various settings, ensuring care aligns with medical and rehabilitation goals. Communicates effectively with patients, families, and interdisciplinary team members to monitor progress and adjust treatments accordingly.

Key Responsibilities

• Assess patients through records, interviews, and observations.
• Create and review personalized treatment plans, instructing patients and caregivers on exercises and home programs.
• Use functional activities to promote health and achieve treatment goals.
• Document evaluations, plans, progress notes, and reports as required.
• Collaborate with team members and attend care conferences.
• Supervise physical therapy assistants, techs, and students to ensure safe, effective patient care.
• Maintain knowledge of equipment use and safety standards.
  
  

Qualifications & Skills

• Active Physical Therapist license in the practice state.
• Effective communication, visual acuity, and ability to work collaboratively.
