- Provide comprehensive primary and acute care to patients in Assisted Living, Memory Care, Hospice, and Group Home settings in Maricopa County.
- Order, interpret, and evaluate diagnostic tests to assess and manage patients’ health conditions.
- Deliver health education, counseling, and disease prevention guidance to promote wellness.
- Collaborate with physicians, nurse practitioners, and other healthcare professionals as part of an interdisciplinary care team.
- Develop, document, and update individualized care plans.
- Prescribe medications and other appropriate treatments in accordance with licensure and scope of practice.
- Requires an MS in Nursing
- Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
- Current unrestricted RN license and NP license in applicable state required.
- Satisfactory completion of a Tuberculosis test is a requirement for this position.
- This position is bonus eligible based on individual and company performance.**
