2. Position Overview
Seeking a Contract Home Health Registered Nurse (RN) to join our team.
- Schedule: 8-hour shifts, 40 hours/week
- Contract Duration: 13 Weeks
- On-call required as needed
3. Key Responsibilities
- Provide skilled nursing care in patients’ homes.
- Complete assessments and OASIS documentation.
- Develop and manage patient care plans.
- Educate patients and families on medications and care.
- Coordinate with interdisciplinary healthcare teams.
4. Requirements
- Active Registered Nurse (RN) License required.
- Minimum 2 years of Home Health experience required.
- Homecare Homebase (HCHB) experience required.
- OASIS experience required.
- Strong communication and organizational skills.
- Must have reliable transportation and valid driver’s license.
5. Education
- Associate Degree in Nursing (ADN) or Nursing Diploma required.
- BSN preferred.
6. Certifications
- Current CPR Certification required.
- Active RN License required.
- CHPN Certification preferred.
7. Preferred Qualifications
- Experience with IV care and wound care.
- Ability to work independently in home settings.
- Flexible schedule and local travel availability.

Pay: $2,000.00 - $2,200.00 per week
