Social Services Assistant - Healthcare Community
At Brandermill Woods, we are committed to providing compassionate, resident-centered care across our continuing care retirement community. We are currently seeking a Social Services Assistant to support the social and emotional well-being of residents in our healthcare community, including skilled nursing and long-term care.
This role is ideal for someone with long-term care experience, who enjoys building relationships, supporting residents and families through transitions, and working collaboratively with an interdisciplinary team.
The Social Services Assistant supports the Social Services department by assisting with admissions, care plans, and ongoing psychosocial support for residents in the healthcare community. This position plays a key role in helping residents and families adjust to changes in care needs while ensuring appropriate resources and support are in place.
Key Responsibilities:
- Welcome new residents and support their adjustment.
- Assist with admissions, transfers, and discharge planning.
- Develop individualized care plans addressing psychosocial needs.
- Conduct comprehensive social histories and assessments.
- Participate in interdisciplinary care planning and resident conferences.
Schedule:
- Part-time: 25 hours per week
- Monday - Friday, with some flexibility
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related Human Services field.
- Previous experience in long-term care, skilled nursing, or a healthcare setting strongly preferred.
- Strong interpersonal, communication, and organizational skills.
- Ability to work collaboratively with residents, families, and interdisciplinary teams.
