- Maintain accurate, timely documentation of services provided

- Active, unrestricted LPN/LVN license in the practicing state
- Current CPR certification (with hands-on component)
- TB skin test completed within the last 12 months
- Reliable transportation
- Preferred: Six months of hands-on nursing experience
