Role Overview: A Physical Therapist (PT) under supervision of Therapy Leadership, responsible for assessing, developing, and implementing individualized treatment plans for adult and geriatric patients in a rehabilitation setting.

- Perform comprehensive evaluations, establish treatment goals, and develop age-appropriate care plans.
- Deliver evidence-based interventions, monitor progress, and adjust treatments accordingly.
- Educate patients and caregivers on exercises, safety, and home programs.
- Ensure accurate documentation, compliance, and timely communication with interdisciplinary teams.
- Participate in professional development, team meetings, and quality improvement initiatives.
- Supervise patients directly, with indirect supervision of technicians and general supervision of PT assistants, in accordance with state practice acts.
- Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
- Current or temporary state licensure, and BLS certification.
- Preferred: 6 months to 1 year of experience with adult and geriatric populations.
- Physical ability to lift up to 50 pounds, stand, walk, and perform hands-on care.
