What are the responsibilities and job description for the Crisis Center Discharge Coordinator position at billingsclinic?
The Discharge Coordinator is responsible for performing a variety of case management activities as specified by the Crisis Center program and the individual client treatment plan. The incumbent acts as an advocate for clients serving as a point of contact and is responsible for assisting clients in linking them to organizations and services outside of the Community Crisis Center. Promotes effective resource utilization; assuming a lead role in providing resources for the clients. This role focuses on achieving optimal client success and linkage to other resources.
Essential Job Functions
• Supports and models behaviors consistent with Billings Clinic’s mission, vision, values, code of business conduct and service expectations. Meets all mandatory organizational and departmental requirements. Maintains competency in all organizational, departmental and outside agency standards as it relates to the environment, employee, patient safety or job performance.
• Conducts or co-facilitates patient groups of an educational or supportive nature according to a patient’s treatment plan and the treatment program.
• Will work in conjunction with the Billings Clinic Psychiatric Bridge Clinic. Will present during Bridge Clinic hours to work on coordination of care for persons receiving these services. Will also provide follow up information for the Bridge Clinic providers.
• Understands and utilizes hospital and community based financial resources and entitlements such as SSD, SSI, Medicaid, charity care, mental health, HCFS, Patient Financial Representatives, Public Health and other outside resources as needed.
• Coordinates and implements discharge/transitional planning activities within expected length of stay in collaboration with the multidisciplinary team. Accountable for appropriate and patient focused, discharge planning; including placement in alternative living environments. Advocates on behalf of patients.
• Coordinates with referral agencies as indicated to ensure services are in place before discharge. This includes providing necessary paperwork in a timely manner in order to process the referral and enhancing positive working relationships with community agencies. Will ensure discharge records are properly received, organized and completed.
• Interfaces appropriately with parents/family to provide support and information in a confidential and therapeutic manner.
• Maintains confidentiality within the treatment team and helps patients to understand confidentiality for themselves and others.
• Utilizes performance improvement principles to assess and improve quality. Collects data as requested for maintaining compliance with quality improvement plan.
• Identifies needs and sets goals for own growth and development; meets all mandatory organizational and departmental requirements. Attends scheduled in-service and education meetings. Attends unit staff meetings and provides ideas and observations to assist in ongoing monitoring and development of the program.
• Maintains competency in all organizational, departmental and outside agency environmental, employee or patient safety standards relevant to job performance. Maintains a safe and clean environment for patients and staff members.
• Performs other duties as assigned or needed to meet the needs of the department/organization.
