What We’re Looking For:
· Strong attention to detail with a commitment to accuracy
· Excellent typing speed and accuracy are essential for success in this position
· Ability to work independently and prioritize effectively in a fast-paced environment
· Customer-service oriented with excellent interpersonal and communication skills
· Working knowledge of brand and generic drug names, uses, doses, and formulations
· High school diploma or equivalent required
· Two (2) years of pharmacy technician experience in a retail or hospital setting/health system required
· National PTCB (CPhT) certification required upon hire
· State of Louisiana Pharmacy Technician Licensure required upon hire
· Pharmacy Technician Career Ladder (Assessment Certificates) required within 60 days of hire
· CPR/BLS certification required within 60 days of hire

What You Will Do:
The Medication History Technician ensures safe and thorough patient care by collecting and documenting comprehensive medication histories for patients being admitted to the hospital.
· Obtain complete and current medication histories from patients through direct interviews, caregiver conversations, and pharmacy or hospital record reviews
· Conduct telephone interviews for upcoming surgery patients to gather medication information
· Communicate with external pharmacies, physicians, and insurance companies to gather drug names, dosages, and administration details
· Enter complete medication history into the hospital's electronic health record for physician review and reconciliation
· Coordinate with nurses, physicians, and case managers to prioritize patients for medication interviews
· Identify and resolve discrepancies in medication histories, escalating as needed to pharmacists or physicians
· Perform quality audits of medication histories to ensure accuracy and completeness
· Assist in the Emergency Department with medication distribution and monthly unit inspections
· Maintain productivity standards in a dynamic and patient-centered environment
