Case Manager - IDDT - Durango Integrated Healthcare, Durango, CO

Position Summary: We are seeking a Case Manager who will participate as a member of a treatment team to design and implement care plans for patients with a wide range of behavioral health conditions. Assists patients in accessing healthcare services and resources to meet healthcare needs, utilizing a trauma-informed, patient centered approach focusing on patient identified goals. Assists patients in improving overall quality of life through improvement in health, relationships, meaningful activities and access to resources.

Integrated Dual Diagnosis Treatment – Member of multidisciplinary team to treat patients with a moderate to severe Substance Use Disorder and co-occurring Mental Health Condition. Services are provided in clinic and in the community.

Qualifications: Our ideal candidate for our Case Manager position will maintain a bachelor’s degree in health or human service-related field; Additionally, they will have a minimum of 1 year in health or human services setting; Preferred 2 years case management experience.

Salary:  Starting pay is $21.00 (Based pay $18.50 $2.50 Locale Differential = $21.00) and ranges up to $23.72 per hour (which includes a $2.50 locale differential at our Durango worksites) and considers preferred experience and preferred education described above.

Physical demands: Employee must occasionally lift and/or move up to 15 pounds. Employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between AXIS HEALTH SYSTEM locations. Possible potential exposure to communicable disease. • Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
• Ability to communicate job-related information to individuals and small groups.
• Ability to work independently with minimal supervision; plan own work schedule effectively and multi-task and prioritize in a fast-paced setting.
• Ability to multi-task, organize and prioritize workload in a sometimes-hectic environment with frequent interruptions.

• Must regard work as confidential.
• Must have current driver’s license, qualify for commercial vehicle insurance and maintain a driving record that allows insurability with Axis Health System’ insurance agency.
• Occasional travel required
• Ability to take on additional duties when necessary.

Additional eligibility requirements:  Annual Flu immunization, Annual TB screening, de-escalation training within 90 days of hire, BLS certification required within 90 days of hire.
