Brief Description
AuthoraCare Collective is currently seeking an LPN Home Health (Skilled home-based-care). This position is full-time: Monday - Friday 8:00am - 5:00pm supporting our High Point, Kernersville and Winston-Salem service areas (office base will be High Point).
Provides high quality nursing services under the direction of a Registered Nurse consistent with the agency's philosophy, policies and standards of care. Responsible for working within the scope of the Nurse Practice Act regarding judgment, nursing skills, teaching, and experience. Works with other members of the interdisciplinary team to meet the patient/family's goals of care. Work with physician to follow medical Plan of Care for the patient.
Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary group and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Assume on call responsibilities as scheduled.

Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. Assume on call responsibilities as scheduled. CPR certification is required.

Requirements
Education and Experience:
- Must be a graduate of an accredited LPN program.
- Must have a current North Carolina LPN license.
- One year's LPN experience is necessary, preferably two years in an acute care, hospice, LTC or home health setting.
- Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
- Strong computer competencies needed including learning and using electronic medical record and documentation software.
- Ability to act with integrity, professionalism, and confidentiality.
- Prioritizing and good organizational skills.
- Patient management skills needed include excellent patient assessment and clinical nursing skills, critical thinking, problem solving, effective interpersonal and communication skills necessary.
- CPR Certification
- Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
Delivery of Patient Care
- Works with IDT to identify patient/family goals of care and establish and maintain the Plan of Care.
- Utilizes nursing process in planning and prioritizing patient care, including assessment and identification of patient/caregiver problems; identification and implementation of interventions; and evaluation of outcomes.
- Establishes therapeutic relationship with patient/caregiver, including providing emotional support and teaching about disease management.
- Recognizes influence of culture, ethics, and beliefs on clinical decision making. Makes home visits in accordance with Plan of Care or at patient.
- Complies with infection control and exposure control policies.
- Works within scope of practice for LPN according to NC Nurse Practice Act. Incorporates changes/advances in nursing practice into own practice. Recognizes influence of culture, ethics and beliefs on clinical decision making.
- Under the direction of the RN in provides treatments, documentation, patient, and caregiver teaching and medication management as directed.
- Contacts RN if needed for a change in the Plan of Care is identified.
- Provides treatments and medications as ordered.
- Responds appropriately to emergency situations.
