AuthoraCare Collective is currently seeking a Community Liaison supporting Durham County (base office will be at our Burlington campus). This is a full-time position Monday - Friday, 8:00am - 5:00pm and requires flexibility to work evenings and weekends and willingness to travel to locations in the community.
Job Summary
The Community Liaison is responsible for outreach, customer service, account management and direct marketing to physicians, advanced care practitioners, case managers, social workers, long term care facilities, and community-based organizations.
The liaison is accountable for developing referral relationships and ensuring smooth, timely access to AuthoraCare programs and services. Promotes services through personal and phone contacts, public presentations, and correspondence. Effectively positions AuthoraCare as the preferred partner. Must be able to work flexible hours, including evenings and weekends if necessary, and able to travel to locations in the community.

Requirements
Education/Qualifications:
- High school diploma or equivalent.
- Bachelor’s degree preferred or equivalent medical sales experience of a minimum of five years and with 2 years of healthcare experience.
- RN, LPN, or SW degree is a plus.
- Three to five years of experience in health care sales or marketing.
- Experience with home care, hospice, long term care or physician relations preferred.
- Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
- Promotes organizational programs and services through consultation and information to physician practices, long term care facilities, outpatient clinics, and community-based organizations.
- Serves as a customer service agent to key referral sources to ensure timely, smooth access to AuthoraCare services. Identifies barriers and takes active role in seeking out and securing solutions to position AuthoraCare as the provider of choice.
- Develops call plans and maintains contacts with existing and new referral sources.
- Utilizes market analytics and data to target key accounts.
- Presents and/or facilitates presentations with other members of the AuthoraCare team to key referral sources on existing services and new program offerings.
- Meets organizational goals and expectations for referrals.
- Maintains referral source tracking reports to include contacts and activities and referral numbers. Ensures that contact information is updated on accounts.
- Assists with the development and updating of agency promotional materials, brochures, and educational information, including the community educational events.
- Maintains understanding of agency programs and regulatory information pertinent to referral management, intake and marketing.
- Provides regular feedback to the AuthoraCare business development team and other staff members on local market trends, referral patterns and customer service needs and issues.
- Assists with the planning of special events, educational workshops and in-services based on referral source and community needs.
- Supports the development and execution of the business development goals and plans.
