Role: IRIS Consultant, supporting individuals to live independently through the IRIS program.

- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining monthly contact and quarterly visits
- Assisting participants with IRIS eligibility and understanding
- Connecting with local agencies to expand service options
- Documenting contacts and visits promptly
- Conducting annual Long-Term Care Functional Screens (as needed)
- Collaborating with a supportive team while working independently
- Bachelor’s degree in social work, psychology, human services, counseling, nursing, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4 years of direct social services experience
- Current IRIS Consultant status or DHS exemption preferred
- Bilingual skills in Spanish, Hmong, Arabic, Russian, or other languages are a plus
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interactions, and community resources
- Willingness to serve diverse populations with empathy
- Valid driver’s license and reliable transportation
- Satisfactory driving record and auto insurance compliance
