Disability Services Consultant - Bachelor's Required

Role: IRIS Consultant, supporting individuals to live independently through the IRIS program.

Responsibilities Include

• Building trusting relationships with participants
• Developing IRIS Support and Services Plans
• Maintaining monthly contact and quarterly visits
• Assisting participants with IRIS eligibility and understanding
• Connecting with local agencies to expand service options
• Documenting contacts and visits promptly
• Conducting annual Long-Term Care Functional Screens (as needed)
• Collaborating with a supportive team while working independently
  
  

Qualifications & Skills

• Bachelor’s degree in social work, psychology, human services, counseling, nursing, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4 years of direct social services experience
• Current IRIS Consultant status or DHS exemption preferred
• Bilingual skills in Spanish, Hmong, Arabic, Russian, or other languages are a plus
• Strong commitment to participant autonomy and respectful service
• Proficiency with smartphones, tablets, and computer systems
• Knowledge of human behavior, social interactions, and community resources
• Willingness to serve diverse populations with empathy

• Valid driver’s license and reliable transportation
• Satisfactory driving record and auto insurance compliance
