- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining regular contact and conducting visits
- Explaining IRIS eligibility and connecting with local agencies
- Documenting interactions accurately and timely
- Potentially conducting annual functional screens
- Collaborating with a supportive team while working independently
- Bachelor’s degree in social work, psychology, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4 years of relevant social services experience, or current IRIS Consultant in good standing
- Bilingual skills a plus
- Strong commitment to participant autonomy and empathy
- Proficiency with smartphones, tablets, and computers
- Knowledge of human behavior, social resources, and community systems
- Willingness to serve diverse populations with respect
- Valid driver’s license and reliable transportation
- Satisfactory driving record and auto insurance compliance
