Job Overview
We are seeking a dynamic and experienced Director of Care Management to lead our Care Management and Social Work Department. This high-impact leadership role ensures quality care coordination, balancing clinical excellence with financial stewardship, while maintaining a strong focus on patient-centered outcomes.
Key Responsibilities
- Team Leadership: Supervise and develop a multidisciplinary team of care managers and social workers to ensure optimal care transitions and coordination.
- Program Oversight: Develop and implement effective care management strategies, including utilization review, discharge planning, and managed care contracting.
- Data-Driven Outcomes: Monitor and evaluate patient care metrics to ensure alignment with clinical best practices and organizational benchmarks.
- Patient Advocacy: Facilitate collaborative treatment planning that prioritizes patient safety, financial efficiency, and quality of life.
- Strategic Alignment: Contribute to organizational strategy by setting and achieving departmental goals related to service excellence, employee engagement, and financial performance.
Qualifications Education
- Required: ADN or BSN
- Preferred: Master’s degree (MHA, MBA, MSN, or related field)
Experience
- Required:
- 10 years of clinical RN experience
- 3 years in case management
- Preferred:
- 2 years in a leadership/management role in care management
Licenses/Certifications
- Required: Active RN license in New Mexico or compact state eligibility
- Preferred: Certified Case Manager (CCM)
