Job Title: Director of Case Management RN
Job Summary:
We are seeking a highly motivated and experienced Director of Case Management RN to oversee our case management department. The Director will be responsible for ensuring optimal patient outcomes, effective utilization of resources, and compliance with all regulatory requirements.
Responsibilities and Duties:
- Lead and manage the case management team to ensure the delivery of high-quality, cost-effective care
- Develop and implement case management programs to meet the needs of patients and optimize healthcare outcomes
- Collaborate with healthcare providers to coordinate patient care and services
- Monitor and evaluate the effectiveness of case management programs and make recommendations for improvement
- Ensure compliance with all relevant regulations and standards
- Develop and maintain relationships with community resources to facilitate patient access to services
- Provide leadership and support to the case management team
Qualifications and Skills:
- Registered Nurse (RN) license
- Bachelor's degree in Nursing, Master's degree preferred
- Certification in Case Management (CCM) preferred
- Minimum of 5 years of experience in case management, with at least 2 years in a leadership role
- Strong leadership and communication skills
- Ability to collaborate with a multidisciplinary team
- Knowledge of healthcare regulations and standards
