Position Summary
We are currently recruiting a PROMISE Care Manager/ Behavioral Case Manager III. This full-time position (37.5 hours per week) pays $23.50 an hour and is located in Newark, DE.
The incumbent will work as part of an interdisciplinary team in the Promise Program serving individuals with severe and persistent mental illness, including those with co-occurring disorders. The incumbent will act as a Care Manager by performing assessments, helping to develop individualized recovery plans, determining appropriate levels of care, placing individuals in those levels of care, and ensuring that care is delivered according to contract standards. Duties will include visiting beneficiaries in the community, charting, and acting as a liaison with numerous community providers. The incumbent may also be asked to cover meetings, represent the office, assist at the front desk to screen calls, and occasionally transport clients in a state vehicle to appointments.

- Perform client assessments and develop individualized recovery plans.
- Determine appropriate levels of care and coordinate client placement.
- Monitor and ensure services are provided according to contractual and program standards.
- Visit beneficiaries in the community to assess progress and support needs.
- Maintain accurate and timely documentation and charting in client records.
- Serve as a liaison with community providers and coordinate care across disciplines.
- Attend and represent the department in meetings as needed.
- Provide occasional assistance at the front desk to answer and screen calls.
- Transport clients to appointments in a state vehicle when required.
- Additional duties as assigned.
- Excellent organizational skills and attention to detail.
- Ability to work independently; must be self-motivated.
- Strong verbal, written, and listening communication skills.
- Ability to understand and adhere to the duties, methods, and procedures required by the position.
- Strong interpersonal and problem-solving skills, with the ability to engage a diverse client population.
- Bachelor’s degree required.
- Experience in the mental health field strongly preferred.
- Valid driver's license required.
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to travel locally for community visits and client appointments.
- Standard hours of work Monday through Friday, 8:30 am – 5:00 pm; however, evening and weekend hours may be required.
