Position Summary

This FULL-TIME position ranges from $16.00-$18.00 per hour depending on experience and is a remote position. The Aftercare Coordinator is responsible for assisting with non-clinical duties for all phases of follow up after inpatient services, and related duties as assigned.

Essential Duties And Responsibilities

  • Follows up on members who are identified by clinical operations as needing aftercare follow up. Verifies compliance with initial follow-up appointments.
  • Provides assistance with referral to a provider if initial appointment is not attended.
  • Assists members in coordinating outpatient follow-up.
  • Checks provider availability.
  • Documents results of follow up calls in system.
  • Completes non-clinical assessment tools relating to after care or HEDIS activities.
  • Calls members and providers and gathers discharge and other non-clinical information from providers/facilities as scripts and workflow allow.
  • Develops and completes reports for completion of outstanding after care activities as needed internally and as requested by clients.
  • Assists and provides non-clinical activities related to HEDIS expectations as determined by quality management leadership.
  • Assists quality management staff with non-clinical work tasks as needed.

Education And Experience Requirements

  • Requires a high school diploma or GED and experience in customer service and/or related field; or any combination of education and experience which would provide an equivalent background.
  • Experience with office software such as Word, Excel, and PowerPoint preferred.
  • BA/BS in health related field preferred.

Physical Requirements

  • Possible prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift and move up to 25 pounds at times.
  • Standard hours of work Monday through Friday, 8:00 am – 5:00 pm.
