- Assessing and developing individualized care plans for residents
- Providing direct care and support to residents with varying levels of need
- Collaborating with physicians and other healthcare professionals to ensure coordination of care
- Communicating effectively with residents, families, and staff to ensure seamless transitions and resolutions
- Participating in quality improvement initiatives to enhance the overall resident experience
