Job Overview

We are seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our dynamic healthcare team. Duties

• Conduct thorough patient assessments to identify social, emotional, and environmental needs, especially in hospice, geriatric, pediatric, and disability populations
• Develop and implement individualized care plans that incorporate community resource coordination, discharge planning, and behavioral health interventions
• Provide clinical counseling including individual, group therapy, and family sessions focused on mental health, addiction counseling, trauma recovery (such as PTSD), and end-of-life care
• Facilitate crisis intervention and manage urgent social issues with sensitivity and professionalism
• Collaborate with healthcare professionals across disciplines to ensure holistic patient care using clinical interprofessional collaboration techniques
• Maintain accurate medical documentation adhering to HIPAA regulations and medical records standards using Electronic Medical Record (EMR) systems
• Engage in clinical research activities related to behavioral health or social work practice improvements when applicable

Requirements

• Master’s degree in Social Work from an accredited program; valid licensure as a Licensed Master Social Worker (LMSW) or equivalent in the relevant state
• Proven experience working with diverse populations including geriatrics, pediatrics, individuals with disabilities or developmental disorders such as autism or dementia
• Knowledge of medical terminology, ICD coding systems (ICD-9/10), CPT coding for billing purposes, and Medicare regulations
• Strong skills in patient assessment, case management, discharge planning, and crisis management
• Familiarity with hospice & palliative medicine practices and end-of-life care protocols
• Ability to work effectively with individuals from various cultural backgrounds in fast-paced healthcare settings such as hospitals or nursing homes
• Proficiency in EMR systems and medical documentation standards; CPR/BLS certification preferred
• Excellent communication skills for client interaction and collaboration within multidisciplinary teams
