Responsibilities:
Coordinate with community-based programs, schools, and childcare centers to connect families with appropriate resources and services.
Guide families through the process of obtaining developmental and educational assessments to identify their child’s needs and necessary interventions.
Advocate for children and their families during Individualized Education Program (IEP) and/or Section 504 Plan meetings.
Provide wrap-around case management services that support families holistically, addressing educational, emotional, and social needs.
Maintain accurate documentation, service plans, and progress notes in accordance with agency and program standards.
Develop and maintain strong partnerships with local service providers, agencies, and educational institutions.
Educate families on their rights, available resources, and strategies for long-term advocacy and success.
Requirements:
Experience working with children with disabilities and their families, preferably within education or social service systems.
Knowledge of IEPs, 504 plans, and Florida’s special education processes and resources.
Strong communication, advocacy, and organizational skills.
Ability to manage multiple cases while maintaining attention to detail and empathy.
Familiarity with local community and family support resources in Central Florida.
