Key Responsibilities
- Assess client needs, barriers, and service eligibility in areas such as housing, employment, education, financial stability, health care, legal needs, family support, and sobriety support.
- Coordinate services with VA personnel, community partners, internal program staff, and external providers to connect clients with appropriate resources.
- Support client intake, maintain confidential case files, complete required documentation, and enter accurate data into HMIS and other required systems.
- Collaborate with clients to develop, monitor, and update Individual Action Plans, Housing Stability Plans, or employment-related plans as applicable.
- Conduct client meetings, home visits, progress documentation, discharge summaries, referrals, and follow-up services in accordance with program requirements.
- Assist with life skills groups, quality management activities, client screenings, reports, team meetings, and other program support functions.
- Pay range: $22 to $25 per hour.
- Job type: Temporary.
