We are seeking a Registered Nurse (RN) Care Manager for a full-time, ongoing role.

- Conduct comprehensive patient assessments and coordinate multidisciplinary care across inpatient, emergency, and outpatient settings.
- Facilitate safe discharge planning, patient and caregiver education, and arrange post-care services.
- Collaborate with healthcare teams to support length-of-stay reduction, readmission prevention, denial reduction, and improve patient satisfaction.
- Perform utilization review activities, including clinical reviews, disposition determination, and managed care variance review.
- Maintain accurate documentation and participate in performance improvement initiatives.
- BSN or MSN degree.
- Current RN license in the state of practice.
- 1–3 years of clinical nursing experience preferred, especially in Emergency, Critical Care, or Medical-Surgical settings.
- Previous case management experience and familiarity with Milliman/InterQual criteria preferred.
- Strong computer skills and knowledge of healthcare financial, regulatory, and payer issues are advantageous.
- Certifications such as PRI within 90 days, and care management credentials (e.g., CCM or CMC) are preferred.
Salary : $124,156 - $158,065
