Position Overview

As a Field RN, you will provide skilled nursing services and in-home assessments while developing individualized care plans and collaborating with caregivers and healthcare teams.

Key Responsibilities:

• Conduct in-home skilled nursing assessments and visits
• Create, manage, and update individualized care plans
• Educate patients and caregivers on conditions, medications, and self-care
• Accurately document all care activities
• Collaborate with interdisciplinary teams for effective care delivery
• Supervise home health aides when required

Schedule:

• Monday through Friday
• 8:00 AM – 5:00 PM
• Field-based travel across Manhattan, NY 

Qualifications:

• Active New York State RN license
• 1–2 years of nursing experience in home care, long-term care, or related setting preferred
• Excellent clinical and communication skills
• Reliable transportation for travel within boroughs
• Bilingual in Spanish or Mandarin is a plus
• Professional liability insurance required
