We are seeking a Licensed Practical Nurse (LPN) to provide professional nursing care in a long-term care setting under the supervision of a registered nurse, advanced practice provider, or physician.

- Assessing residents and administering medications or treatments as ordered
- Monitoring resident health, comfort, and safety, and responding to emergent situations
- Documenting care interactions, outcomes, and resident progress
- Collaborating effectively with residents, families, healthcare providers, and interdisciplinary team members
- Counseling residents and families on prevention and treatment plans
- Performing testing and evaluating data to reflect resident status
- Supporting resident education and self-care programs
- Graduate from an accredited practical nursing program; associate degree preferred
- Current unencumbered LPN license with multi-state licensure privileges
- Experience in clinical settings, including supervised clinical hours or a minimum of 400 hours as an LPN
- Additional credentials for IV therapy management if applicable
- Ability to maintain confidentiality, communicate clearly, and work effectively within a team
- Commitment to ongoing education and competency requirements
