Role Overview: The Licensed Practical Nurse (LPN) provides professional nursing care to residents in long-term care settings, under supervision of registered nurses, advanced practice providers, or physicians.

- Assessing residents and administering medications or treatments as ordered
- Monitoring resident health, comfort, and safety
- Documenting care interactions, outcomes, and resident responses
- Recognizing problems, recommending solutions, and responding to emergent situations
- Collaborating with healthcare team members and establishing effective relationships
- Counseling residents and families on prevention and treatment plans
- Supporting resident education and developing self-care programs
- Performing additional duties as assigned
- Graduate from an accredited practical nursing program; associate degree preferred
- Valid LPN license with multi-state privileges
- Experience or clinical training across the lifespan; minimum of 400 hours as an LPN preferred
- Competency in IV therapy (if required by location)
- Ability to communicate clearly, maintain confidentiality, and work effectively within an interdisciplinary team
- Completion of ongoing education and certifications as required
